Payments and Insurance

Brookfield Clinics, at its Garden City location, accepts most insurances, including PPO's and some HMO's. As a courtesy to our new patients, our staff will contact your insurance company to determine your benefits for treatment at our facility. If you want to give us a call before your first scheduled appointment, we will advise you of any deductible and co-pays.

We also offer a secure online form to schedule an appointment. You will also be asked about insurance information and we can verify the insurance ahead of time.
